These are Individual Health Care Plans that are being shared by a UFAN family who developed these plans with their allergist (for their daughter) and they are now being used throughout their school district. You may use them as a template or for reference material when developing your child's own plan.

We also announced that UFAN received a small grant from FAAN and will be hosting a breakfast in October with the Utah Department of Health (DOH) and are inviting key state education representatives and stakeholders to inform them about the new laws from H.B. 56, the need for this information to be disseminated throughout the schools, and about the training and forms that are being developed by the DOH.
